Would you recommend Kybella for my armpit area?

Thanks for your question, Snickerdoodle123 (and great profile name!) The real question is whether that is a fatty deposit or what is called ectopic breast tissue, that is, breast tissue that is the underarm as a result of embryologic development. Sometimes we can determine which it is based on some questions: was it there when you were at a lower weight? is the area responsive to weight loss? does it change with your menstrual cycle or pregnancy? does the site feel like other areas in the body with fat deposits or is it more firm? does the area get sensitive or tender? Only after these questions and a physical exam can real treatment recommendations be made. If we believe the site is breast gland, then a surgical solution is usually recommended, though the relevant procedures are often minor. If the area is felt to be fat, then kybella, coolsculpting, focal liposuction, or excision can be entertained, and relative assets and liabilities of each approach can be discussed. When stay at home orders are lifted, I would encourage a formal consultation. Good luck.

Would both a horizontal and vertical incision get me flatness?

Congratulations on your weight loss! I would agree with your assessment that what remains is loose skin, not stubborn fat. But I would also agree with my colleagues that a traditional abdominoplasty approach with a transverse incision should achieve your results. I don't believe the addition of a vertical incision would add significantly to your result, but would come at the expense of a significant scar. When stay at home orders are lifted, I would encourage you to get a formal consult with a Board Certified Plastic Surgeon, not only with an interest in weight loss body contouring, but with considerable experience treating male patients. My approach to male patients is different, as I find the shape of traditional tummy tuck incisions feminizing, so I alter the incision. How can I tighten the skin on my neck?

Thanks for the inquiry, Nat1972. It's difficult to give a definitive response based on an image, and when stay at home measures are relaxed, I would encourage a consultation with a BCPS. The real question is whether you have frankly excess skin, or lax skin. If you have excess skin, a surgical approach would be recommended as none of the skin tightening technologies are strong enough to address that problem. If you have lax skin with bands, then you may want to consider some of the modalities available - from ultherapy, to renuvion, to exilis - possibly combining treatment with fat, filler, or a biostimulant for the creases.
