You have a mild to moderate case of gynecomastia . Plus your skin appears to be in good shape so you should expect to get a good result with a standard small incision gynecomastia correction surgery. This would consist of liposuction to reduce the fatty component and then a small peri areolar incision to remove excess sub areolar breast tissue. This small scar typically heals well and this outpatient surgery has a very quick recovery.
you appear to have a minor degree of gynecomastia. Because most of your " puffiness" is in the nipple areola area it is likely to be mostly breast tissue as opposed to fatty tissue. Although most cases of gynecomastia correction are done with a combination of liposuction and direct excision of the excess thick breast tissue, most of the breast tissue is in the sub areolar area which cannot be removed by liposuction. That component needs direct excision, usually done through a small periareolar incision. This means that liposuction alone ( or coolsculpting for that matter) is unlikely to correct your gynecomastia even though your case is mild.
It appears that you have some definite skin laxity , especially in the lower abdomen. If you have not had pregnancies it would appear that you have lost a lot of weight ( particularly if you have stretch marks). You probably are best suited for an abdominoplasty but a " mini" abdominoplasty with liposuction would definitely give you a nice improvement. You could add a skin tightening procedure such as the J Plasma procedure to add to the results at the same time.
