Undereye filler is a tricky thing - the hill under your left eye is likely residual filler and should be melted. Then reassess with a clean slate.
Full disclosure: the surgeon likes to operate, while the medspa likes to inject, hence they're selling you want they like to do. From your pics, it looks like cheek volume would benefit you the most. The deep plane facelift surgery would address some of the cheek volume by repositioning the larger fat pads upward and back toward your cheekbone, but fat transfer at the same time can really help your cheeks be the highlight! The benefits of fat transfer over filler is that you pay for the procedure, not for every syringe, and that your fat is your own tissue.
I see many young patients who say "droopy" or "saggy." If you've had recent weight loss or are thin build to begin with, a subtle volumizer would be a great option. At this point, consider PLLA (Sculptra) to help you with your concern. Likely you'll need more than one treatment, and it takes a while to see measurable results. Good news is it can last a long time and your skin looks even better!
