Thank you for your question. This is a common question that many surgeons are asked by patients seeking augmentation or revision. The breast has a specific footprint on the chest and tissue characteristics that are unique to every individual. An implant must be located based on that footprint, otherwise significant distortion and a terribly unattractive look is the result. Based on your pictures, an implant with the correct width and projection, placed in an appropriate position, may give you improved cleavage at the risk of looking a little fake, with which you indicated you are ok. Having clear discussions about expectations and finding a surgeon with significant experience who can guide you on your results while being frank about the possible trade-offs and complications (such as malposition, synmastia, rippling, palpability, thinning of tissues over time, bottoming out) based on your anatomy and tissue characteristics is paramount. Ask questions, be sure that you understand the answers, and do not hesitate to get a couple of opinions from board certified plastic surgeons. Best of luck.
Congratulations on your tummy tuck and thank you for your question. Based on the appearance of your incision your surgery seems to be quite recent. In an attempt to limit the length of the scar, shortening the upper part if the incision so that it matched the lower part, creates the pleats. It does not appear that you have "dog ears." Remember, your body wants to be back to normal and it will do what it needs to get as close to that as possible. The pleats will typically resolve within three to four months. Scar management, starting about two weeks after surgery when the surface closure has started to stabilize, can help move the healing along. Quality silicone gel sheets can make a big difference in the overall scar appearance. Topical products, daily massage and certain lasers can also have a significant effect. Nutrition and providing your body what it needs to facilitate healing is critical. Scar revision is always an option once the healing is complete. All these should be discussed with your surgeon, as well as any concerns. You should feel comfortable in the relationship with your surgeon to help you through the healing process, which is a number of months. Remember that your surgeon and his/her staff are there to support you and only want the best outcome for you.
Congratulations on your recent surgery. Remember that recovery is a process. Asymmetry and distortion are not uncommon and typically resolve quickly with time as your body adapts to the changes. Your surgeon wants the best possible outcome for you. This is a relationship and having the trust to keep an open dialogue is critical. Do not hesitate to voice your concerns and be open to the explanations and advice. Best of luck for a smooth recovery.
